Check list for I-485 for EB category

I got the list from a friend and not sure if it is missing anything - may be those who already filed can help out.

The I-485 form lists the following evidence as the initial evidence for the employment-based I-485 filings:
1. Birth Certificate (Affidavit from family members, if 'BC' is not issued at birth)
2. Copy of passport page(s) with Non immigrant visa (All pages to be safe)
3. Photos (??)
4. Biometrics (currently after filing) - is this same as finger printing?
5. I-693 Medical report for all applicants
6. G-325A Biographic Information Sheet(s)
7. Evidence of status for complete stay in US which include I-20 (if any) + I-94 + current status evidence + copies of H1B approval notices with I-94 attached (in case you get RFE) + copy of OPT (if any) - *important.
8. Sponsoring employer letter
9. I-140 petition Receipt or Approval Notice
10. I-485 Receipt Notice or Approval Notice, if spouse filing separately. If filing jointly not required.
11. Marriage certificate, marriage termination evidence if filed as a spouse, and birth certificate if filed as a child
12. Other eligibility documentation (??)
13. I have heard RFE in few cases for W2s and/or individual Tax Returns for all years in US. This is probably for additional tracking purposes and 'not' A2P.
14. A notarized self-deceleration stmt that you are not a murder, rapist, spy or terrorist with affidavits from your family and at least two co-workers and a letter from local police that you are in fact behaving well. (Don't panic! I am just kidding. This is not needed. I didn't want to end the list at superstitious no 13).

let me try to categorize:

**FORM with 2 photograph ****write name and A# if u have at the back

to prove eligibility for 485:

primary:
1. only I-140 approval or receipt notice
depenent:
1. only I-140 approval or receipt notice of primary
2. marriage certificate (or something establishing relationship)
3. I-864 or I-134 (not mandatory, but my university immig person made me send I-864 even though that form is not for employment based category)
4. i sent: tax returns, recent w-2, 2 recent pay stubs/slips of primary to prove income (because of I-864)

to prove legal status:
1. all past immigration papers, I-94, H1B/ F1/J1 etc...
2. color copy of non-immigrant visa (blown to full page, not must, i did it)
3. color copy of all pages of passport, just in case, i did not send because university staff suggested, but some people got RFE

information about applicant:

1. g325a form, all 4 copies
2. passport biographic page (blown up to full page)
3. birth certificate
4. 2-3 recent pay slips
5. recent w-2
6. I-693 Medical report for all applicants
7. Employment Verification letter with salary mentioned, with both primary and dependent
8. for dependent: 485 receipt notice or approval notice if not filing together

I think on interview they check if any doc is missing in I-485 file.
We were asked on interview to submit:
- W-2 and tax returnt to complete them for last 3 years
- 3 last pay stubs from both me and spouse (when I asked, if paid bi-weekly do you want 6 of them? they replied, no, does not matter, just give 3 last...)
- fresh employment letter
- they wanted my daughter's birth certificate in everybody's file (mine and spouses) and we submitted only one in my daughters file... we did not have extra copies but the officer was friendly and did those copies from the one she had...
- if you had proir arrests, then submit certified court docs on each.
 
i am filing for i-485.my spouse is on h-4,what are documents to prove h-4 status.is h-4 visa on passport and i-94 enough?or is there any specific document for h-4?
 
i hope valid h-4 visa on the passport and i-94 are the only documents.i hope there no specific h-4 papers as such like h-1 papers.
