I want to change my first name only and keep my last name, should I just simply fill out the name change in part one? I mean do I need to provide any documentation regarding that?

you dont need to provide any documentation. be aware that if you request name change, you wont be able, in case there is same day oath, to make it at that ceremony. You ll be rescheduled.
