Just send a letter to the head of the credentialing committee stating: 'As I have stopped practicing in hicktown, KN, I no longer require priviledges at 'hicktown catholic memorial family victory medical center'. The care of my patients has been transferred to Dr mybossforwaytoolong. I allways enjoyed practicing at HCMFVMC and I wish you and the hospital the best for the future.'
And here is why. If you don't relinquish your priviledges, they might cancel them for something like missing 50% of comittee meetings, not admitting enough patients or for failing to fill out some forms they sent you. If years from now you have to provide this hospital as a reference for another credentialing body, they will mark an 'x' in the box that says 'where this physicians priviledges ever restricted or cancelled'.
(also make sure you completed all your charts, not doing so creates potential liability for you and the hospital)
