Change in contract while doing waiver

Hi all,
I had been putting all my hopes on getting GC via perm. However, I am from a retrogression country and dont want to put all my eggs in 1 basket. I want to file for NIW as well.

Basic info: Started J1 waiver on July 1st 2005 (conrad 30) , sponsored as hem onc.

Complications in my case as I see it
1) I am sponsored as Hem-Onc.
2) My contract is for 3 years only.
3) My wage is slighltly lower than the prevailing wage.

I am thinking of getting a new contract with my employer stating that I will be working as Hematologist/Oncologist AND Internist, contract duration will be 5 years with a clasue of termination after 3 years, without cause if either party wishes. I will also try to have him increase my pay to meet prevailing wage.

Do you think after making those changes, I may have a decent shot at NIW?
Also, assuming my employer agrees to all these, which agencies will have to be informed? Probably the state department of health who sponsored my conrad 30 waiver, but what about USCIS? Should I inform them prior to making changes and see what there response is?
