Certification of good conduct

I got a letter from INS to submit a certification of good conduct for my mom to complete her I485 process. Anybody knows what should be the formate of the certificate? Also should I get it from with in US also? She is here for almost over 1 year now.

Yesterday I got the same letter for my in-laws. What did you do for that?
Did she had to get a letter from you home country too?
