Case Status Online - whats happened?

I filed I-360 petition in May 2003. In May 2004 received RFE. INS got my evidence on June 11, so there was a note online:
“On June 11, 2004, we received your response to our request for evidence or information. It is taking between 90 and 300 days for us to process this kind of case. However because preliminary processing was complete, the remaining processing time will be less than the maximum stated in this message. You will receive a written decision on this case.”

After that I waited for 3 months, made status inquiries. Yesterday i saw this online: ”On November 8, 2004, we received your response to our request for evidence or information. ...........”

How to explain this date change? Whats happened? I did not send them anything in November!
 
"On November 19, 2004, the document we made based on the approval or registration of this case was mailed directly to the person to whom issued"
This is I-131 case
What does this message mean? I received RFE for picture in October and they received that on Nov 4, 2004. Received another RFE for picture, id, and I-94 on Nov 9, 2004 Which I HAVE NOT YET SENT.
Now this message, can someone please help me understand?
