I (finally) decided to go ahead and apply for the case#(DS3035),and I have a few questions regarding the actual application:
1.How safe is to apply with out the lawyer( Section 9) ?

2.Section 6 : weather my exchange program(residency/fellowship training) include US gov. funds? (I presume “yes”)

3.Section 11: address of correspondence: current(valid until the end of the training 6/2007) vs. attorney(that I plan to hire) .

4.Section 12: list all exchange visitor programs….a)program number?(is that the actual number that I can find @ FREIDA/AMA?)
b)purpose of the form?
c)funding amount ?
5 Section 18: Alien registration number?(I dont think I have one)

6. Among the other documents that I have to submit along with the above application, I was asked to provide a “Statement demonstrating” why im eligible for the waiver....
Any advice /sample of this statement /letter?(j1doctor.com was a great resource in the past but it is not working any more…)

Have you successfully filed for your own taxes ?
It is an individual decision whether you want to use a lawyer for this, I did it with one because my practice paid for it, but if you think you are up to it and understand the process, there is certainly no secret behind it.

Yes. Your residency hospital recieved anywhere from 90-150k per year from medicare specifically for training you.

but funny enough, the entry on my form was 'NO'

I would 'make' a permanent adrress by renting a mailbox at the UPS store for this purpose (just put 'suite X' instead of the official PMB X. )

That is the USIA program number for the entire ECFMG EVSP. You can find that on your IAP66/DS2019.(I'll look it up on my dos3035 and get back to you)
addendum Ihad N0000392136 in the first column and P-3-04510 in the second. purpose: 'medical training' and subject 60.0215 funding n/a
(some of this might be specialty specific, so double check please).

unless you have applied for a greencard in the past you wont have one

that you fulfilled all the rules of the j1 program and that you are planning to obtain state health department sponsorship for a practice in a underserved location.

1.JUST TO MAKE SURE,WHAT I SHOULD CHECK FOR THE SECTION 5(ds3035) FOR THE CONRAD 30 PATH.:
A)Exceptional Hardship
B)Interested Government Agency (non-physician) State Health Agency Request
C)Interested Government Agency(Physician)
D)No Objection Statement
E)Persecution
