Case For Lc From 2001, Don't Know How To Check

I have filed for PLC with RIR back in March 2001. Since then I or my employer didn't receive any letter, case # etc. The only sign I got that they have the file was when two years ago an DOL officer called in the office and asked my employer if he would agree with the new prevailing wage.
Since 2001 I've changed my address twice but the employer address and phone # are still the same.
DOES SOMEBODY KNOW WHERE AND HOW I CAN CHECK ABOUT THE STATUS OF MY CASE

The employer can inquire with the state workforce agency (best in writing). It is the employer who files, so they can get information about the case.

Is there any standards for the inquiry or he can send free-form letter?
 
I would write it as a free-form letter, include your name DOB, SSN as well as the employer identification number.
The HR person should state that they are still interested in employing you and that they would like to know what stage the process is in.

(check in the labor cert forums, a lot of these old cases have been sent to 'backlog elimination centers' which have procedures on how to get information)
