Canceled without prejudice

Hi,
My niece attended the f1 visa interview 3 days back. Officer told the visa is approved and will mail the visa. After getting the passport the visa sticker was present but stamped CANCELED WITHOUT PREJUDICE.
Can any one help me understand this. Is the visa approved or rejected.
 
It seems the visa was initially approved and a sticker placed in the passport. However, that decision was reversed and the issued visa was cancelled. There is no other letter/form/instructions with the returned passport?
