Can extend H1 visa after 6 year term

Hi,

My wife is doing her 2nd year medical residency on H1B visa. She is going to complete 6 year H visa term by end of 2006 Feb (because she was on H4 for 3.5 years). After 2006 Feb, she is going to be shortage of 6 months visa to complete her residency. Is there any way she can extend her H1 visa for another 6 months to 1 year or is there any way she get the Green Card on future employement/NIW basis.

CAT 22 situation, without completing residency and getting the license to practise, labor certification for GC is not practical.
