BirthCertificate - name missing !

Hi Guys,
I got my 140 approval and going to apply for 485/AOS. I have birth certificate issues for both - myself and my wife.

Me - DOB is different from the one in all my records. + My Name is missing.

Wife - Name is missing.

Just providing affidavits with the original BC will suffice? or do I need to provide NON-Availability Certificate as well(Doesnt make sense though as BC is available but have problems)

If I get the name added to the BC and get a new BC, should my wifes BC contain her maiden name ? or her currentname?. How to work that out?

Any light on these issues would be highly appreciated.

--- do you mean your passport has diffrent DOB and your Birth certificate has diffrent DOB
Wife - Name is missing.
--- where the name is missing? on her birth certificate?

---need to know what is wrong on your BC, explain with details or give example of your actual name( provide any name for example) and the name and dob as appears on BC and on passport
or do I need to provide NON-Availability Certificate as well(Doesnt make sense though as BC is available but have problems)

-------- that will be ok if your DOB and name match with your passport info(same name and same DOB)
should my wifes BC contain her maiden name ?
---- what is her name on passport? if she changed name on passport after marriage then marriage certificate and name change affidevit will be enough
or her currentname?. How to work that out?

DOb problem in birth certificate..

Thanks for the reply. I am elaborating in detail on the problem.

My actual DOB is 11/xx/19xx but its 09/xx/19xx in all my records(Passport/10th Cert..) So for all practical purposes, my DOB is 09/20/19xx. Now problem is that DOb in birth registration records/birth certificate is 11/xx/19xx

So would providing a affidavit would suffice in this case?

Problem with my Wife(Maiden - Kavita Verma | After Marriage - Kavita Kapoor) birth certificate is that her name is missing in that. I am trying to have her name added to her birthcertificate. Should we add "Kavita - First name" / "Kavita verma-maiden name" / "Kavita Kapoor - Aftermarriage name" She got her name changed in passport and and we do have a marriage certificate and name change affidavit.
