Birth Certificate Unavailability Format?

I do not have a birth certificate and believe that I need to get affidavits from my parents and a certificate from the municipality that my birth was not registered. I have the format for the affidavits but not for the unavailability of birth certificate. Can anyone please see a format?

Municipality will have printed format of non-availability certificate you don’t need to provide then format. If the non-availability certificate is not in English get it translated by Notary in India on stamp paper.

Problem is 99% municipalities in my home country (India) does not have a form for non-availability certificate for birth registration.

In north India, Gujarat and Maharashtra the municipality or registrar of birth office have the non-availability certificate printed in Indian language.
