Birth certificate - Un available need sample format for affadivit

I am planning to sponsor my parents for GC. They were asking for Birth certificate. Birth certificate is not available.

I know, I can have 1 affadivit for unavilabilty of Birth certificate and another affadivit for Birth.

Can any one provide me sample of Birth certificate affadivit?

Note: Almost no one who saw my Dad's Birth are alive.My Dad is 70 + so no elder brothers or parents are alive.

get an affidavit from 2 people who your dad knows for a long time. In the affidavit mention the persons DOB and he states that he knows your dad and he was born on 01.01.1000. Get it notarised and it should work, i did it for my dad and it worked.
