Birth Certificate issue - 485

My name in Birth Certificate has been recorded differently (different name) than my school and other official records.

As part of 485 documents should we submit any form of affidavit or secondary documents. Please advise.

get Non-availability of birth certificate and 2 affidavits from parents with your name as on passport, Dob, as on passport place of birth as on passport. dont submit the BC that has your name wrong on it
 
To add to what ginnu said...please make sure that the alternative name is mentioned in the Biographic form 325. In that way, you will be doubly covered.

This was an advise from my attorney. I myself have 3 middle names in my birthcertificate and my passport, but the rest of my documentation has just my last name and first name, here in US.

If his name on passport is Ramesh and on birth certificate it is Ramnesh (different name) he should not submit that BC and NOT write that on form G385A as he never used that name his all education certificate and passport are Ramesh.

On G325A write only the names you used to write on education certificates or passport or visa stamp. If it is lady and her name before marriage is different and after marriage changed to married name then she need to write both the names

What about the situation where there is a discrepancy with Date of Birth ?
A person was born on Dec 1st lets say, and lets say the records have it as June 1st in the passport etc...

How should one go about with the Birth Certificate in that regard ?
Please advise as a friend of mine is in that situation.

Please help, I have submitted the BC which was registered in 2003 got the following RFE... I need to respond asap its been more than month since I received the RFE

Please submit the oldest available evidence which establishes your birth and parentage. This evidence may
include, but is not limited to:
a. Hospital birth records which name the child and both parents.
b. Medical records which name the child and both parents.
c. School records which name the child and both parents.
d. Census records which name the child and both parents.
e. Religious records in the form of a certificate under the seal of the organization where the
baptism, dedication, presentation or comparable rite occurred following the birth, showing
the date and place of the child's birth, the date of the religious ceremony, and the
names of the child's parents.
