Birth Certificate does not have name`

Hi GC gurus,
My fiance\'s birth certificate does not have her name but it has names of her parents. Can anyone of you post a format of the affidavit to be taken from her parents. Thanks.

You have to get three affidavtis each from her parents and one from a close relative.each shud have full name and address of the person,and age of the person and if possbile the job..then shud state that so and so is my first or second daugther and shud state she is my daughter.Also, since at the birth time it was not mandatory to have registred with the name,we did not do so..something like that
IF possible you can get the same from where the birth was registered or register her birth with the name...
