Birth Certificate Affidavits ?

Hello Friends,

I need to get my and my wife\'s Birth affidavits. Is anyone familiar with the process ?

1. Can anyone recommed a standard foramt ?
2. Do we need to get them from our parents or any other relative ?
3. Do our parents need to get them on a stamp paper in India ?

I really appreciate any help on this.

I have a birth certificate issued by hospital but it does not have my name ...pl advise

Won\'t do. Should be from local Municipal/govt agency entrusted with this job

For Missing details, get two older relatives to file affidavits.
