Birth Certificate affidavit

My original birth certificate issued by the municipality does not have my name in it.
So I got one affidavit signed by my mom. Unfortunately my mom expired last year.
Can I use this affidavit or do I need a new one?

Do I need 2 affidavits or is one sufficient?

You have to have 2 affidavits ..it can be from any relative like your uncle, aunty etc,. so get another one and you can use the one you have...
