3. (Y) was born on to mother (X1) and father (X2) , who were married to each other at the time. He was born in, (place of birth including country name)
4. To my knowledge, (Y) birth was not recorded by any government agency or religious group at the time of birth.
5. I am personally familiar with facts of the birth of (Y)
because (I was present at the time of birth etc. etc)

Here affidavit of birth is for person Y
X1 and X2 are mother and father of Y
X (may be father or mother in that case X=X2 or X=X1) is signing the affidavit
Conditions
1. X should be eqaul to more than 15 years older than Y
2. In order to provide substitute of original birth certificate this affidavit must be notorized and there should be two separate affidavits from two realtives
3. INS will be satisfaied if you obtain a certificate of non availability of birth record from municipality of the place where you were born in your country
