My birth was registered only last year. So I have to get affidavits from my parents. Does anyone know if mother's maiden name needs to be on these affidavits?
your mother can mention her Maiden name also but the present name is MUST. they need present name
XYX was born to me on date.... at City/village in the state of....
My name before marriage was...... and at the time of i gave birth to my Son XYX my name was.... becase in India married ladies normaly change the name after marriage.
