Best way to Submit documents

I wanted to know whats the best format to submit NIW docments.
1] do you write cover letter (saying whats attached etc.) with I-485 and I-765 forms?
2] Can you put tabs and number the attachments especially for I-140 which has numerous attachments?
3] Can you use binder clips for documents related to one form together?
Those who have successfully submitted please comment/suggest.

For the I-140, I did as advised in many places around:
- Punched the whole file at the top (two holes and a metal flat binding thing)
- Used tabs at the bottom on stronger paper at the beginning of the memo itself and at the beginning of each bunch of exhibits that I grouped by criterion.
- What shows up on top is a one page cover letter presenting the whole document.

The idea is to make the reader's life easy.

As for the I-485 and related forms (I-131, I-765...), it seems better not to punch anything. Better to use paper clips to separate documents. Indeed, a cover letter for each of the forms is a good idea! The first thing that will happen is that the various forms will actually be looked at separately, unlike your I-140 file...
