B2 Extension-Require Original I-94 and is it okay to apply if you have 28 Days remaining of yourstay

Hi,
We are thinking to apply for my mom's visitor visa extension for 4 months. Her vaild stay on I-94 is till Oct-24th 2012.
We are planning to visit Canada from Sept.21-Sept.27.
1. Is it advisable to travel after you apply for an extension?
2. Is it necessary to submit your original I-94 with the application?
3. Is it okay to apply for an extension after coming back from Canada trip which leaves 28 days from her vaild stay expires and not 45 days(which on the USCIS website,they suggest to apply atleast 45 days before)?
Please help.

A1: Yes
A2: I believe the instructions do say that but people still submit a copy only
A3: If you can submit before 45 days, do so

Keep in mind the most important aspect of a B-2 based EOS, the justification for an extension. Ideally it should be a life/death emergency. Anything that could have been addressed with a better timed initial entry is not an emergency.
