B1 Visa stamping issue at port of entry

Hello everybody. I am first time member and I take this opportunity to thank all the great contributors and advisors for all our immigration issues. Its a wonderful site and enjoyed learning several facts about immigration.

I am currently having issue with my mothers B1 visa. After three attempts at it in Madras embassy she actually got the visa this year (unfortunately my father didnt live long enough to witness this, he was there for the earlier two rejections) with nothing changed on her front or for the fact that I am the only child (which apparently was a reason to reject her earlier application).

So now she has 10yr visa issued and she entered chicago on 25th OCT. 2008 and at the port of entry she handed the blue immigration form and the officer stamped her passport but forgot to write the date she should return (the space is left empty).
I need help in resolving the issue.
1) whom should I contact to get the stamping corrected (any immigration office or do I need to go to Chicago?)

what about the white form? on which the I-94 is attached? she had to fill it as she is a visitor, you mean to say she never got the I-94?

She does have the I94 (white form) filled and attached to her passport. Just the date is missing.
