B1/B2 Visa Rejected for family

I have been living in USA for approx 4 years, my family (mom, dad, brother) went for their B1/B2 Visitor Visa & it got rejected; please provide your input/suggestions. Below is the interview Q & A of the entire interview;

Q; Your older son is in USA?
A; Yes he is in USA, since approx 4 years.

Q; What is he doing there?
A; He went for his MBA and now working on H1-B.

Q; Counselor asked general questions about father's business (income, turnover) mom, younger brother?
A; All the relevant answers were provided.

Q; Do you have anyone else in your family?
A; Parent's said that they are just a family of 4. (Parents and 2 sons)

Q; Counselor looks at the screen and asked the same question again, did you say that you just have 2 sons?
A; Yes we just have 2 sons.

and then the counselor hands over the passports saying, I am sorry I can't grant you non-immigrant visa.

When I came to USA 4 years ago, I filled in DS-156, DS-157 and DS-158, within DS-158 I entered my sibling’s information, back then we were 3 brothers including me, but due to a car accident in my family, my middle brother passed away, since then my parents generally say that they have 2 sons. Having the same in mind when asked by the counselor my parents answered the same that just they have 2 sons (older in USA and younger one next to them).

We think that this might be the reason that their visa got rejected. As per the Consulates records they had information that my parents have 3 children instead 2. Therefore, I wanted your suggestions on how shall we proceed our case, as this was a mistake they did unknowingly & it lead to mismatch of records. I am really looking forward for them to visit for 3 weeks.

Please provide your inputs/suggestions.I will look forward for your response.
 
I do not think it was due to the mismatch of the records. Were they given any paperwork with the rejection? If it states 214(b), then your parents were rejected due to the fact that they could not overcome 214(b).
If they were rejected under 212(a)(6)(C), then it was due to fraud and/or misrepresentation.

By the way,

Consul = officer dealing with consular affairs in the consulate/embassy
Counselor = officer working in the political wing of the embassy

Triple Citizen makes a very good point. Typically, you will be given some information so you can identify where you went wrong (or where they feel you went wrong). That way you can correct it and have it taken care of the next time. Were you not provided any such information? I also doubt that it had to do with the sad loss of your family member, but mistakes like this are fairly common.
 
They should also be prepared to answer that since your entry there was the tragic loss of your brother if again asked about the family composition.
