Go to your nearest SSA office and show them your documentation (passport, stamp, etc.) They will look you up on their computer system, and if you are already entered as a LPR, then they will order the card for you. It takes about a week or so to arrive. However, there is a chance that the database with your new status has not been updated, in which case they will not process your SSN until it is.

As is the case for my wife, 4 weeks and waiting. The local office says it can take up to 10 weeks.
