The AOS application may be one of the most important applications you fill out in your life. Incorrectly entered information can set your application process back weeks, or even cause your application to be rejected. Here are some hints to make sure your application is processed in a correct and thorough manner.
When possible, please type or use a computer to fill out any government application. If you must handwrite it, make sure that you write in block letters and that each letter and number is legible. DO NOT WRITE IN SCRIPT, SLANTING OR CURSIVE WRITING. DO NOT USE A PENCIL OR DRY PEN. When you are finished filling out the form, give it to a family member to see if they have a problem reading it. If something is illegible, white it out and write over it, or start again with a fresh form.
PART 1
Family Name/Given Name/Middle Name This is the MOST IMPORTANT PART of the application. It is imperative that all field are filled correctly, and legibly. Always, on all forms, enter your last name first, with your last name in CAPITAL LETTERS. If Jose Alfredo Ramirez were filling out this form, he would enter RAMIREZ / Jose / Alfredo (or "A."). If Mr. Ramirez had entered his name as "Jose Alfredo Ramirez", "Jose" would be captured as his last name, "Alfredo" would be captured as his first name and "Ramirez" would be captured as his middle name.
Address c/o field Very important - this field can ONLY contain the name of an individual or firm. It cannot contain an address. If you want all materials sent to your lawyer's office, you must use their office address to fill in the address fields below. If you put your law firm's address in the C/O field and your home address in the address fields, you forms will be mailed to your home address!
Zip Code It is extremely important that your correct five digit zip code is entered here. If less than five digits are entered, or if the wrong zip code is typed in, there will be delays in processing your application!
County of Birth and Citizenship/Nationality Make sure you fill in BOTH of these fields! You just need to write in the name of the country for both fields. Please make sure that the place you enter is a recognized national body; "West Indies", "Central America", or "Africa" are unacceptable entries and will result in your application being delayed. Please type the entire name of your country. Do not put MX for Mexico, DR for Dominican Republic or likewise.
A # This is an eight or nine digit number.
I-94 #This is an eleven digit number.
Current USCIS Status If you entered this country illegally, put "Entry Without Inspection" or "Without Inspection." If you are in the country on a valid tourist visa, enter "Visitor for Pleasure" or "Visitor for Business" accordingly. If you are on a student visa, put "Academic Student" or "Vocational Student" according to the terms of your visa.
PART 3
Your Mother's/Father's Name Please only put your mother's or father's first name here. Two names at most should be entered in each field.
Current Occupation If this form is being filed for a child under 16, put "Children under 16".
Place of Last Entry It is important that you list both the city AND state that your entered in. If you put the city's name without a state, it will not be captured. AIRPORT NAMES OR ABBERVIATIONS ARE NOT ACCEPTABLE and will not be captured. If you entered at JFK, put "New York, NY." If you entered at Dulles, put "Washington, D.C." *The sole exception is Baltimore-Washington International Airport, Maryland. If you entered at the border crossing in Tijuana, put "San Ysidro, CA". If you entered at the border crossing at Agua Prieto, Mexico, put "Douglas, AZ."
Gender, Marital Status and Permanent Residence Please make sure you only check off ONE of each of the boxes in this field. If you check the wrong box by mistake, circle the correct box to indicate the entry you want inputed.
PART 4
Signature Don't forget to sign the last page! Failure to do so may result in your application being rejected or delayed. Don't forget to print your name CLEARLY next to your signature. This might be referred to if your name in part 1 was illegible or poorly scanned.
Remember to have a family member look over your forms to make sure they are legible. It may sound silly, but a clear, properly filled out form may make a difference in the speed which which your case is handled.
