Assistance related to 485 filling

Since so many of you in this form has filed self petition without a lawyer. I would appreciate your help related to my 485 filling.
I have 3 questions:
1- Should I include all my old I-20 or DS-2019 or only the current one??
I was here on F-1 (2000-2004), went home, got a tourist visa which I used to come to the US for a conference (10 days) then started my J-1 (july 2006-current).
Am I suppose to provide a paragraph of explanation of the above situation, with copies of my visas, I-20 and passport stamps (no old I-94's)???

My spouse also was here on a J-1 (2000-2002), then on F-2 for 4 months in 2003. He was at our home country (Apr. 2004-july 2006) and then on J-2 (july 2006-current). I would like to point out here that he was subjected to the 2 year home requirment during his first J-1, but I assume that it is ok since he already spent 2 years there. SHould I also explain all this? If I do not, and just included copies of all the old forms, how would the USCIS know that we were in a legal status all the time?
Sorry for my naive question!!

2- Who signs a minor dependant 485 form? Should I sign it (part 4 ) amd write my name next to it or the depandant's name??? SHould I also fill part 5 (person preparing the form if other than the above)????

3- LAst question, I have sent an advisory opinion request but have not received a response yet, can I send the application without and when they issue an RFE, I should be able to have it by then??

I am not sure about signing for minors, i think you can do it most of the cases.
You have mentioned that you are in J1(July 2006-Current), You cannot file 485 if you are subjected to 212(E). If you are applying for waiver, make sure your waiver includes your spouse too.
 
I beleive my current J-1 is not subjected to the 212(E), that is why I am applying to 485. I am currently waiting for the advisory opinion letter to inlcude with my application.
Any thoughts about the first question??

We are trying to get a copy of my husband's old IAP-66. I have all the rest my ealier F-1 and his F-2 but not IAP-66. We are trying to contact the University and hoping that they kept a copy of it in their storage files.

Does any one know where else can we find a copy of it?? What happens if we send the application without it? would a copy of his old J-1 visa be enough??

I will also include a summary of visa status.

I do not have copies of my old I-94, is this a problem??

This is a naiive question, but all the arrival stamps to my home country on my passport are not in English. and we do not get stamped when leaving the country, is this an issue?? PLease excuse my ignorance on this matter.

Wondering if anyone can help me with the second question??
