AR11 for Spouse?

Does my wife also required to file AR11?

Can somebody tell what should I fill for following in AR11 form.
1) Status in US
2) Alien number
3) My stay in US expires on

Every adult has to file AR-11 when they change residence.

1) Status in US: write visa classification or adjustee if 485 pending.
2) Alien number: Write none if you do not have one.
3) If adjustee write "adjustee" or else I-94 date.
