AR 11 questions After GC

Hi all
I got plastic card during last week of sept. now I have moved from LA to NY.
I need to send AR 11 form. But I am sure what to fill in following fields of form AR11

1. PORT of Entry INTO US

During pass stamping I returned my last I-94 and we do not have that any more. My last I-94 had the following info:
POE ---> SFO
POE Date --> Feb 5, 2004

----
Information on I--551 shows
POE --> ATL ( which happened to be my POE 3 years back when I applied for I-140)
POE Date --> August 12
I-485 approval date --> August 18, 2004
Stamping date -->SFO on Aug 25

So what should be I write in this field in AR11 for POE details?

2. Copy Number from ALIEN CARD: Is this the same as A#.

Please suggest?

I think you should go with the latest I-94 info. 'cause thats what CIS will have in the database. Your stamping is possibly a mistake. I need to check mine can post in the evening.

The above info should go to your AR-11.
 
I needed to file AR-11 two weeks after stamping, and my lawyer told me to use the POE and DOE (same as I-485 approval) info from the stamp, because adjustment is technically considered an "entry".

What do you mean by "you have to put the information on your I551 stamp."?

Do you mean even after you have the card ? or until you get the card?

more over, how can you tamper the I551 stamp ? :confused:
