AR-11 - late submission? port/date of entry?

Hi-
Moved few weeks ago and discovered that I've missed the 10 day cut off for submitting AR-11! Is this a big problem?????

I'm a permanent resident and don't know what to answer for the fields which require me to enter my "port of entry into the U.S." and "date of entry..." Are these the date and service center where I was granted residency? Or I don't need to fill them out since I'm a PR? Or was it my last entry into the US?

Please share your experience. GC holder lost the job and also planning to move to different location. Will send the AR11 within 10 days. However, gonna a problem since he will leave it blank for the question work address ?

Does that mean a person who gets a re-entry permit for a temporary visit abroad have to file an AR-11 with a work address in a foreign country?
