AR-11 Form Questions

HI,

I have a couple of questions regarding the AR-11 form.

1) Date of Entry into US...does this mean the last date of entry?

2) I work on EAD and am waiting for I-485 approval..so what is my status? The options given are: Visitor, Permanent Resident, Student, Others (Pls specify)..What should I write?

3) My stay in US expires on ...should I give the expiration date on EAD?

Any inputs would be helpful.

==> Other - Adjustment of Status

==> Leave it blank.

What sertra has said correct for 1 and 2. For 3 rd question, instead of blank you can put "I485 Pending" or "Pending AOS"
