AR-11 form q (related to address change before oath)

i had my interview this morning and the IO approved my case for citizenship. however, because of a name change, they will need to schedule my oath date. meanwhile, i will be moving to another local address soon. i brought this up with the IO and she said to mail the change of address along with the N-652 (interview results) to the same DO.

so i'm unsure what to enter on the new AR-11 form for the following fields given the sort of in-between status i'm in:

a) country of citizenship - from what i understand i am not a US citizen until i take the oath and am naturalized, correct?
b) along the same lines, I am in the US as Visitor/Permanent resident/Student - i am a PR until after the oath, right?
c) Port of entry into the US & date - not sure what i am to enter here.

applied for police clearances on: 11/17/09 & recd by 12/12/09 (whew!)
passed test & approved for citizenship :) 12/15/09 (oath to be admin by a judge due to name change so could not be done on the same day.)

You are not in any in-between status. You are a PR until you become a USC.

You are not a U.S citizen so you list your current nationality. Also the port of entry, is the last place of entry.
