Nice to hear sweet voice \'This case has been approved....\' here are my details.

btw .. wife case approved too.

Started with regular labor (january 1998)
changed to RIR (some time around early 2000)
EB2
PD : Jan 98
I-485
RD : Dec 15,2000
ND : Dec 20,2000
fp : APRIL,2000
LIN # 01-061-5XXXXX
AD : ???? (checked AVM yesterday .. approved on 29th )
will post more once I get the papers.
no rfe (sent yellow form )
country : INDIA

where do I get yellow form on net

I have my FP scheduled next week. do i have to submit this yellow form along with it(even if not asked).please let me know what you did in your case. and congrats.

They did sent me an yellow form with fp notice..
I dont know where you can get the yellow form on the net..

I send (marriage certificate, employment letter, and h1 papers)
along with the yellow form.

I dont know what to suggest.. if I were you I will send the above papers even if there is no yellow form.. with your alien # on top..
while some suggest not to send anything until INS asks for..
ultimately its yr decision.

i think i got the answer

I did some research of my own after i posted the question and got this link

it says that the yellow form (IRS 9003) is no longer needed now. also by looking att the form i remember i submitted it along with my 485 application.

IRS 9003 is not the yellow form

The yellow form comes with the FP notice asking for verification of employment and EAD etc.. It also says it is not required to be filed but will help speed up the case. You cannot get a yellow form and file it unless INS had sent it to you

what is the number of this yellow form

if it is a INS for there has to be a number (G-### or I-### etc.) Do you know what is it? Thanks for your clarification anyway.
 
Yellow form

Here\'s what I know of Yellow form : this I read somewhere in immitracer

Yellow form is issued by NSC along with FP notice to the applicant and attorney. The submission of yellow form is not mandatory.
(no numbers on the form)

The intention of the yellow form is to allow EB-485 applicants to submit proactively evidence with the NSC normally requires in RFE so that EB-485 application is adjudicated with out issuing RFE.

NSC will send yellow form with FP notice, listing documents/evidence it will require to speed up completion of 485 adjudications.

to submit with yellow form:

employment letter
EAD documents

The above two requirements are clearly stated in the yellow form
additionally we can submit H1 and I-94 copies.. which may help.

The top of the form says:

Pending Form I-485(EB) - Please Interfile Immediately with file:
A_______________________

the yellow form also says ... there is no requirement that you respond to this request for information.
