Applyting For H1-b Visa I-129

My company is applying for my h1b visa. I am transfering from an F-1 visa to H-1B. I am currently working under my OPT (expires in September 2002). I am filling out the H1-B form. In the basis for classification heading there are 4 choices:
a] New Employment
b] Continuation d previously approved employemnt without change
c] Change in previously approved employment
d] New concurrent employment

Which of these 4 classifications would I fall under. I believe (c) as i am changing from OPT to an H1-B...change of status!!
THereafter I have to fill out another item Prior Petition number- would this be the EAD card number or the A# on my EAD? (I guess this is the account number)
please advice!!
 
Generally OPT is not company specific but H-1B is

So most likely it will be [a] New Employment for you unless you have a prior H1-B before F1 status. In any case you are applying for a Change of Status. Part 2, Item 4 on the application form.

Again can be prior H1-B petition number.

But if you answer New Employment prior petition is not needed
