Applying for Visa Revalidation (DS-156, 157) with Duplicate Passport - Any Issues?

Gurus,

I am applying for my H1B visa revalidation in US. I had my original passport lost in Jun 1999. Got a duploicate passport and my H1 stamp is in the newly issued duplicate passport.
Now, for Revalidation, DS-157 asks if my passport has ever been lost and if I say yes to it, what additional information should I submit?
A police report ? Anything else that would be of help?

------------- Provide the copy of police report and attach separate sheet and explain where/when you lost the PP in details and circumstances, provide the lost passport # and issue date, country, expiry date. They want to be sure that some other person may have not used that PP with visa on it to enter US. They will check the records and if a HIT then they may not issue you visa or delay in Visa stamp. If your previous passport that you lost did NOT had US Visa stamp then I don’t think of any problem
