Applying for OCI, please help

Hello all,
I am about to send my OCI application to the New York office (resident of MA). NY office does not seem to ask for any extra photograph. Here is a list of items being sent in the application packet.

* Two sets of the following.
- Page 1, 2 & 3 of the online registration form (Page1 - Part A, Page2-3: Part B).
- Remaining pages, i.e. 4, 5, 6 are instructions only (not sending these).
- One 35x35mm photo attached on each Part A form.
- First (photo and other information) page of the US passport, notarized.
- First (photo and other information) page of the Indian passport, notarized.

* $275.00 "certified cheque" payable to the "Consulate General of India, New York".


Question 1: Does the list look complete?
Question 2: I plan to send the $20.00 for mailing charges when I send my passport. Is that the right time or should I send it now?
Question 3: My Indian passport is not cancelled yet, should I send it later when the OCI application is approved?
Your help is greatly appreciated, thanks in advance.

I sent in an extra photograph - total 3, also copy of my naturalization certificate.

Once your OCI process is complete and the documents have been received at CGINY then you send in both passports - US/Indian and $20 for return mailing.
