All copies should be self-attested with a statement that 'This is a true copy of the original' and your signature.

Do we have to write this statement on all the pages or just the second set of copies. Thanks

I did it on all pages. You never know when consulate might return your packet saying, "it's not self attested."
The OCI guy at Travisa said I need to make 2 more copies of my last 2 pages of the Indian passport. I asked him if he wants me to write the true copy bullshit.. He said, "It's just a formality, no need for it." Hence, I didn't on the last 2 copies.

I wrote it wherever I could find space. For example on copy of the Naturalization Certificate, there isn't much space. So I wrote it in capital letters on the side margins that are less than .5 .. I made it squeeze in there and it fit.
On copies of the passport, I wrote it horizontal. On certain papers where space was little, I wrote it vertical.
On the rest of the copies, I got tired of writing the same thing over and over again so after the 4th page I started writing in cursive and cut my signature in half ... There wasn't any problem with that. My OCI processed just fine. Now I wait for it to get to the SF Consulate.
