Applying for EAD

All, I'm applying for an EAD and my I-485 is being processed.

On the form, it is asking me if I have ever applied for an EAD before? I had applied for EAD when I finished my MS on an F1 visa (for the OPT).

Should I say "yes" to this question? I don't have my OPT card with me now.

Is there a way for me to get a duplicate after four years??

I also had the same question when I applied for EAD. Nobody answered.

So, I put "Yes" in the 765 form and gave details. I had my old EAD with me though and eclosed a copy. Haven't heard back from USCIS on the case yet.
