Applying for a new VISA stamp after a previous B1 STATUS extention? what is expected?

Hi,

I am in the process of applying for an extention of my B1 status inside USA.
My reason is to have more observership period in a hospital which is mysituation currently. I am a physician.

Will this affect me negatively if I decided to apply to a new B1 VISA at a US consulate outside in the future? In other words, will extention of B1 status, hardens getting a new B1 stamp in the future or getting any other VISAs ?

Do you advise me that instead of applying for extention I depart from US and apply for a new VISA outside directly and without waiting a coniderable time outside? Please be note that my current visa is not valid for more than a 2 months more from now.
 
Unless it is a life/death emergency, I would advise you not to file for an extension.

So If I want to have a more 6 months period, you advise me to depart and apply for a new visa outside ? better than extention?
