Applying AP on my own

I am applying my AP on my own today. My first AP expired last month. I just had a doubt. Do I also need to send a proof of my current status. I mean an H1 or EAD copy along with the 485/140 copies.

I would appreciate response of anyone who has filed AP on his own. Secondly do u have the CSC Laguna offices mailing address as I need to fedex the docs today.
