Applied I-131 online.. Now need to send supporting docs

I filed for EAD renewal 4 months back and got it approved 2 months back.
Now I have applied for advance parole online.
I need to send supporting documents now.
What all docs should I send?
Also , do I need to attach a letter stating that I have go to my native place etc etc... If yes, does anyone has the format of that letter?

In I-131 receipt, there is no mention of appointment for fingerprinting or photo etc.. So I haven't taken one..
Is this Ok?

My I-485 application is pending since March'04.

You dont need FP for AP. The list of documents to be submitted are mentioned in the instructions section of I-131.

This is a sample format i found online and improvised a bit.
