hi i have receive a congratulations leters from a kentucky
i have to complete a form DS230 part 1 and 2
and they have send me two for each one
i'm married i'm asking if i should to complete the second one for my husband

you need to fill out form DS-230 for yourself and all qualifying members of your family who will be going with you (spouse and unmarried kids under 21). You need to fill out form DSP-122 only for the actual winner (yourself?).

Kindly let us know about thepost date of your notification letter and the receipt date and the country where you receieved your notification.
