you can try going to the INS field office nearby you live.
Also take wifes Medical records just incase they need to verify.

The CIS may want to see the clearance from the doctor(s) who performed the surgery that she is OK to fly.

After two weeks she should be able to travel ,and getting a letter from the doctor wont be a problem thanks again.
