Here's the checklist for emergency AP at NYC office.

- Form 131
- Fee 110
- Two color photos
- Proof of status (140/485/EAD)
- Reason for travel (Hospital letter w/ translation)
- Birth certificate

Get in line before 7:00am so that you can in front of RAP unit window on 8th floor by 11:00AM. After that, you need Death Certificate. For immeciate family member only: parent, children, brother, sister. Grand parent is NOT.

Questions:

- I receive two copies, one To Alien with photo, one To Transportation Lines w/o photo. Am I suppose to give the origional or just copy of the second copy to airline on the way back? Why I heard somebody mentioned 3?

- I should give all I-94s to airline on the way out, right? Who fill out the back of I-94s, airline or me?

- Besides passport, AP paper, 140 approval, 485 receipt, EAD, what else do I need to carry with me?

- Somebody mentioned getting a new I-94, is that given at the port of entry?

Anything else you guys can advice me? Thank you very much.

Good post.
Some dish out 3 , some do 2. All have photos , only one has the photo. It only means that each ASC director has discretion to print whatever comes to their mind. No big deal.

Just submit the I94s. they should staple it with your ticket/boarding pass portion. Latest I94 is enough.

Dont offer I485 receipt, I140 receipt and EAD unless asked for. Fill out new I94 and submit passport, AP, and I94 to the POE inspector. You will be taken for secondary inspection.

You fill the new I94. Get passport, AP document and I94 stamped as Adjustee or Parolee
