AP Renewal-Queries???, Please help & pls reply!!!

I am about to renew AP for me and my wife and have the following questions

SOMEONE WHO HAS DONE PAPER AP APPLICATION RECENTLY PLEASE DO REPLY!!!!

1. Is birth certificate really needed for AP application???

2. In the AP application instruction on USCIS website they had mentioned that ear should be visible, I thought that they had changed and they are now accepting standard PP size photos, please clarify???

3. i live in Ohio where should i be mailing the AP documents considering tha I will be using FEDEX.

4. My I140 is approved and I485 pending since 2004 and I have changed jobs using AC21 but not filed for AC21 now when I send my H1B approval notice it has the name of my new employer(not the one who sponsored my gc), will this cause any issues or any rfes?

5. Can somebody Please email me the standard cover/supporting letter that we send along with the application.

-------------- with AP papers you dont need to send H1 approval notice copy (even if it has new employer name it is not going to harm. GC is for future job)

----------------- you should write your cover letter for your AP with your pending I-485 reciept# your A# your DOB, your address and what you have attached with form you can post it here. if any corrections others will suggest.
