AP questions, please help

Gurus,

I have the following questions to fill my wife's AP.

1) In I-131, what is Class of Admission (part-1, question-3) ? What should we fill ?
2) In part-3, question-1 : Date of Intended Departure, what should we fill as we haven't planned the trip yet. We are only applying AP for emergency purpose.
3) What should we write in a seperate sheet of paper for justifying AP ? What supporting docs needs to be submitted ?

----- you will not find the answer in I-131 instrustions many are confused by this and many lawyer are also confused, best thing to do is write H4 if your wife last time entred on H4. AND on cover letter write:
Class Of Admission: My last entry to US was on H4 or H1and my I-485 is pending to adjust my status
2) In part-3, question-1 : Date of Intended Departure, what should we fill as we haven't planned the trip yet. We are only applying AP for emergency purpose.
-- write Any date after 1 month they dont care
3) What should we write in a seperate sheet of paper for justifying AP ?
--Any reason, like visiting family or mother or father they dont care
What supporting docs needs to be submitted ?
I-485 filing reciept, copy of I-94 both the sides, 2 photographs ( PASSPORT STYLE) Fees $ 165, I-131 form, COLOR copy of the first page of passport that has name, DOB and Photo, Color copy of the Driving licence or State ID and marriage certificate copy, if she was issed the AP in Past then send the copy of that AP
