AP filing help please?

hi! Gurus,

Do you guys point me to sample cover letter for AP filing..I am in AC21 case, dont want to take chances.. with expired I-94..

I am planning to send

1) $110
2) 2 ADIT photo
3) I-131
4) I-485 ND xerox
5) with Addendum for FORM-131 that explaing part 7 of I-131

Please help me to locate sample coverletter, should i send copy of passport and I-94 in which date is expired???

131 Application Part 7

Hi,
In part 7 of Application form for AP, they ask
"On a separate sheet of paper, please explain how you qualify for AP and what circumstances warrant issuance of AP. Include copies of any documents you wish considered."

Do applicants usually add a sheet explaining this?
