AP/EAD Renewal - Urgent Info needed

Gurus,

I want to apply for my paper based EAD/AP renewal's based on I-485 emloyment based(EB3) pending in vermont center and needed to confirm few things from you guys.

1) Should the checks for both AP and EAD be payable to "U.S. Citizenship and
immigration Services."?
2) For my spouse's EAD and AP should i send only one copy of her I-1485 receipt notice or i should make two copies.The reason i am asking is in the cover letter for each application for EAD/AP i am saying that I-485 receipt notice is attached. Since she is filing for EAD/AP does she require two copies of I-485 receipt notices to enclose in each set for EAD/AP or just one should be sufficient?
3) In the form I-765, in point number 9 (asking for SSN number if used any).Should i give my wife's SSN.?
My wife has an EAD and got her SSN in FEB-2005 and she's not working.She has just used it once in filing joint tax returns with me although she has no earnings.
4) Do we require front facing latest PP size photos (issued by USCIS) for EAD and AP both?
5) Apart from DL and PP , is there anything else required in color copy?
6) For AP do i have to write a suppliment letter for Part 7?Is it compulsory?

I sent one with the EAD and one with the AP

YES

Yes. You need to put something down. All I said i was going to visit family who I had not seen for 2 years, for a few weeks

Can you send me the format of the suppliment letter for AP.I would appreciate that.
 
I did not do a formal letter.
All I did is put the date and said exactly this.

I am planning on visiting my family who I have not seen in two years for a few weeks in the summer.
