AP and I-94

I'm going to travel home using Advance Parole. I have question about filling I-94 form. There are questions like

8. Country where you live

What should I write here? My home country or USA?

10. City where visa was issued

I received AP from CSC. Should I write Laguna Niguel?

1. U can use US address (you are waiting to get the GC you remember..)
2. Write BCIS or department or not applicable

Where did you see the questions? I am also planning to travel on AP and was wondering what type questions, I have to answer.
Is there a website where I can look at a copy of I-94. Please let me know.
