AOS - which forms to fill out

Guys, i just need some advice on which forms to fill out. I have done most research, but fill me in if there are any gaps in my research.

Situation: My father as an LPR (in 2004) submitted an I-130 for me. I-130 was approved in 2005
My father became US citizen in 2010 and I am in US on H1B. I just checked june 2010 visa bulletin and my priority date is current as of June 1st, 2010.

I want to go ahead and process an AOS (ie. adjustment of status within USA).

So, according to my research. I should submit:
1) I-130 (receipt and approval notice)
2) Letter requesting change of category from F2B to F1 (based on father being US Citizen - photocopy of his passport)
3) I-485 (adjustment of status)
4) I-765 (Employment authorization form)
5) I-131 (Advance parole form)
6) I-864 (Affidavit of Support, filed by petitioner)
7) I-693 (medical form)
8) G-1145 (electronic notification form)
9) submit them to Chicago lockbox on June 1st.

Is there anything I am missing?
 
Ok another question: Since i never requested a change of category from F2B to F1 (based on my father becoming US Citizen in january)?

What are the special requirements to do this? Should i do anything about this or just send along approved I-130 with a copy of his US passport with I-485?

Any new criteria or paperwork that i need to fill out?

Please advice guys. I sincerely need anyone's and everyone's help.

Just send a copy of the I-130 approval notice with a copy of your father's passport. That should be sufficient.
 
question about forms

TheRealCanadian: so about the forms. Can you suggest any other form that i must submit with this application or those are good enough. I under you're processed under EB-3 category, but if you have any experience in family based, then pls suggest
 
Don't forget the application fees. This may seem obvious but you won't believe how many people fall for such mistake)

Guys quick question regarding: I-765 application
Q.13) Place of last entry into US.

I am canadian citizen and my last entry was from Alberta. This is where my immigration was processed. Should i write Alberta? or where i landed in US (ie. Altlanta, GA)?

Q.16) Really really confusing. For the life of me, i can't figure out the Category that i classify under. I am doing Adjustment of status on family based immigration -- Am i under C9 category?
