anyone with 06/2001 RD still no RFE and no approval?

If your spouse had an RFE sometimes they link the cases together. In my case I did not have an RFE(06/01/2001), but my wife had a BC RFE. My approval did not go through.

Both of us were approved after a week after my wifes RFE was submitted.

RD:06/01/2001
ND:07/20/2001
Wife RFE: 06/04/2002
RFE Submitted and Recd by INS:08/20/2002
Approval for Both:08/26/2002

Another reason could be if your I140 has been approved for CP.
